Support Worker (Reach)

Shipley/Community based across the Bradford District

Reach is part of the health and social care offer for people with Serious Mental Illness (SMI) across Bradford, Airedale and Wharfedale.

We are looking for experienced Support Workers to join our Reach team working alongside Community Mental Health Teams (CMHTs) to support clients with SMI to reach their goals.

You would manage a caseload of clients 1:1 helping them to develop their own plans and goals with consideration and in step with wider support.

Our work makes it easier for people to understand and navigate the support options which will make the biggest difference to them. It helps people gain confidence, build resilience, develop skills, and get involved in activities to enable a more full, happy, and healthier life.

This is a challenging and rewarding role – as well as working with people with serious mental illness you would be part of The Cellar Trust team and work closely with colleagues in the health system and across community settings. Due to the nature of the work, it would be desirable if applicants can speak Urdu/Punjabi to enable them to understand instructions and communicate effectively with the individuals we support.

Community Mental Health Support Worker (CORE)

Shipley / Bradford District

We are looking for a Community Mental Health Support Worker to deliver peer support to clients accessing CORE. CORE is a service in partnership with BDCFT to build a bridge between primary and secondary care, working in the community with lead providers across specific primary care networks: Affinity, 5 Parks and North & West Primary Care Network (PCN).

You will provide one-to-one and group peer support sessions to individuals with Complex and Severe Mental Illness (SMI) accessing our services in line with their needs and service specification. You will work closely with members of the multidisciplinary team including mental health professions and external stakeholders, to assist individuals in accessing resources and other support networks to enhance their overall well-being and recovery. You will have an experience of effectively managing a client caseload, working with people with mental health difficulties and challenging behaviour and an understanding of the barriers they face.

Crisis Support Worker – outreach (Safe Spaces)

Bradford and Keighley (with outreach across Bradford District and Craven)

We are looking for dedicated and compassionate individuals to join our Safe Spaces team as Crisis Support Workers. In this role, you will be responsible for delivering vital outreach crisis support to children, young people, and families (CYPF) across the Bradford District and Craven area. You will provide immediate, in-the-moment crisis support to children, young people, and families experiencing mental health distress and offer outreach support in a place that suits the individual such as school or at home. You should have a sound understanding of how to support people in mental distress and be able to use various approaches to support individuals to manage the feelings of crisis. You will have experience of working with vulnerable people or, with children and young people and be passionate about helping people.
