Community Mental Health Support Worker (CORE)

Shipley / Bradford District

We are looking for a Community Mental Health Support Worker to deliver peer support to clients accessing CORE. CORE is a service in partnership with BDCFT to build a bridge between primary and secondary care, working in the community with lead providers across specific primary care networks: Affinity, 5 Parks and North & West Primary Care Network (PCN).

You will provide one-to-one and group peer support sessions to individuals with Complex and Severe Mental Illness (SMI) accessing our services in line with their needs and service specification. You will work closely with members of the multidisciplinary team including mental health professions and external stakeholders, to assist individuals in accessing resources and other support networks to enhance their overall well-being and recovery. You will have an experience of effectively managing a client caseload, working with people with mental health difficulties and challenging behaviour and an understanding of the barriers they face. Working closely with our Leadership Team, you will implement impactful client involvement strategies that address community needs and drive quality improvements. Your role will be instrumental in building trusted relationships within local communities, fostering an inclusive environment, and increasing referrals from underserved communities.
